CONSUMER ACCESS TO MENTAL & BEHAVIORAL HEALTH RECORDS AND COMPLAINT INFORMATION

This notice is provided in accordance with Texas Health & Safety Code §181.105, as enacted by House Bill 4224 (89th Regular Session).

HOW TO REQUEST YOUR BEHAVIORAL HEALTH RECORDS

Under Texas law, you have the right to access and obtain copies of your mental and behavioral health records, subject to applicable state and federal laws.

To request your records:

• Submit a written request to our Records Department or Privacy Officer
• Include your full name, date of birth, and current contact information
• Specify the records requested (such as dates of service or type of record)
• Requests may be submitted in person, by mail, or email.
